Portal Registration

 

In order to ensure privacy, a patient is invited to use our portal via email and is given a secure link.  If you are a patient who would like to use our portal, please call us at 570-421-5123, choose option 3, and give us your email address.  An email invitation will be sent to you with a link to the registration page.  Please fill out the information correctly.  You will be given a user name and asked to create a password. When registration is complete you may choose the "Patient Portal Login" link from our website to access the portal.
